A one-page checklist should be filled out each time a sharp/needle is found in an inappropriate location, left unattended or disposed of improperly. The same form is used to report a needlestick injury. The SHC Environmental Health and Safety Department explained that the form is a tool designed to track and consequently reduce needle stick injuries. For questions, further information or to obtain forms, contact the EH&S Department at (650) 723-3177. Forms are also available on most patient care units.
