Profiles added to physician credentialing

A Medical Staff Activity Profile is now part of each medical staff member's credentialing file to help provide objective information for Stanford Hospital and Clinics' re-credentialing process. The profiles, implemented in December, provide one-page summary and additional detail information in five general areas:

Utilization Management
Focus Review (formerly called "Peer Review")
Medical Record Audit Outcomes
Transfusion Services
Pharmacy Services

The reports will be compiled for all medical staff members, said Sandi Edgar, director of medical staff services. (Fellows and house staff are not medical staff members and therefore aren't included.)

"The goal is to make the credentialing process more objective, to ensure that physicians are all evaluated using relevant, factual criteria and to ensure that we are making decisions using the best measures and information possible," said Larry Shuer, chief of staff. "Additionally, the material in aggregate will provide us with useful data and benchmarks that will help us make the hospital safer and more efficient," Shuer added.

The summary page will include statistics on inpatient and clinic usage, average length of stay of admitted patients, a summary of cases reviewed by peers and the Care Review Committee, as well as the number of cases found outside the criteria of care. A transfusion services summary will show total cases, as well as the total number of cases that fall outside of criteria.

Medical record issues, such as number of illegible handwriting orders issued and unapproved abbreviations, will be noted. A pharmacy services summary will reflect the total orders for a particular drug or product, as well as the number of orders not meeting established institutional criteria.

"A key goal of the process," said Shuer, "is to reflect as many denominators as possible, so that busy physicians who treat a high volume of complex patients will not be penalized," the chief of staff said.

A detailed report in each file will show the data and characteristics of each element noted statistically in the summary
