Geriatric Health Services has been established at SHC to coordinate care and provide specialized services for older adults. The program, under the auspices of the Guest Services department, promotes comprehensive care for older adults with a collaborative effort in planning, implementing and coordinating appointments, physician referrals, assessments and testing, advocacy, continuum of care at home and other special geriatric services, said the service's director, Rita Ghatak. "We're offering a contact point that older adults can use to find the special help they need to access hospital resources and find specific tools to live as safely and independently as possible, Ghatak said. The Geriatric program also manages Partners in Caring, Lifeline and Vial of Life - three longstanding programs under SHC Guest Services. Partners in Caring (winner of Stanford University's 2004 Community Partnership Award) provides trained volunteers who give practical, non-medical assistance for older adults. The program is a partnership with Avenidas, a Palo Alto community service agency. Lifeline has been a service of Stanford Hospital for 20 years and is an in-home emergency response system designed to enhance independence among older adults. Vial of Life is a program to provide first responders, such as paramedics, with easy access to crucial medical history information when they arrive at a home emergency.
