Farewell to Falls supports community elderly

The Trauma Service has created a Farewell to Falls home-based program designed to help older adults - specifically those who have recently fallen - maintain their independence.

Farewell to Falls is a cooperative venture involving the Palo Alto and Menlo Park Fire Departments, SHC's Emergency Department, and community physicians.

The program focuses on medication review, exercise, and home modification as well as the senior's health history. It consists of two home-based visits by an occupational therapist with follow-up phone calls and an annual evaluation visit.
